In the portal, you will have access to your own electronic medical record. This will show different visits that you have had at Lincoln County Hospital.
For each visit to our facility, you will have a Summary of Care Document. This document summarizes the care that you received while you were a patient in the hospital.
Different information included in the Summary of Care Document includes:
After each visit or admission to the hospital or emergency department, you can go online and login to your Patient Portal to view a new Summary of Care Document for each visit. These documents can be accessed any time after you are discharged from the facility.
When registering as a patient at the hospital, our registration staff will ask you for your email address, which will allow our system to send you an email invitation containing a link to your Patient Portal.

Creating an Account
Once you have provided your email address you will receive an email from Lincoln County Hospital providing you with a link to create your user account.
Click on the link provided in your email to start the registration process.
Fill in the following information:
The Profile # and email address will automatically be filled in for you.
Next, create a User ID and password, select the checkbox verifying the information is correct, and Click on Register!
The next step will have you select 3 security questions and answers that will be used in the event that you forget your password.
Start by going to: thrivepatientportal.com
After logging in with your User ID and password, click View Clinical Information.
Next, select the applicable patient to view. (If you are listed as an authorized representative for more than one account, you will see all available patients listed here.)
Select the applicable account, all accounts for your visits at LCH will be listed here. You can recognize each visit by your admission/discharge date from the facility.
This will include information from your visit such as test results, medications, allergies, immunizations, and health issues.
*at this time the “Pay Your Bill” feature is currently not in use
